Complete the Application

If you have gone through all the webpages, you should have no problem filling out the application! Nearly all of the questions on the application were covered on the webpages!

We recommend that you save the application on your desktop so you know where to find it. Click HERE for instructions to save your application.

  • Once your file is saved, when you open it in Adobe Acrobat*, you will be able to type information directly into each field. Use the tab button or click with your mouse to maneuver through the fields.

  • Remember to save your file often as you enter information, so that you do not lose the data you’ve entered.


Ways to get the documents, forms, and photos to Compassionate Care Network

1. If you want to continue with the application digitally, you can do the following:

  • scan your documents and save them

  • download the REVIVE application and the Contractor Selection Confirmation form (if you have more than 1 contractor you will need 2 copies of this)

  • open up each form one at a time in Adobe Acrobat*

  • fill the form out online, sign it digitally, and repeat for each form

  • email them to us at ccnincHRG@gmail.com along with copies of your scanned documents and photos.

2. Or if the “old-school” way is best, you can do this:

  • collect your paper documents and other forms

  • download the REVIVE application and the Contractor Selection Confirmation form,

  • print the REVIVE application and the Contractor Selection Confirmation form (twice if you have 2 contractors you’ve selected)

  • fill them out with a pen, sign them, and

  • send them to us along with your documents and photos via U.S. mail at Compassionate Care Network, 3300 Hess Ave, Saginaw MI 48601.

3. We also have a secure drop box on the left side of the front entrance of Lewis Temple Church at 3300 Hess Ave., where you can drop off your documents, forms, and photos. Please put them all together in a large envelope with your name, telephone number, and email address on it.
