2026 Forms
REVIVE application - (2 pages) for common repairs needed over time in the life of a home (roof, siding, windows, furnace, water heater, etc.), and for eligible repairs and modifications to a home to make it more accessible and to assist with aging in place and independent living (ramp, hand rails, widened doorways, grab bars, bathroom modifications, etc.)- $15,000 maximum.
REVIVE application + more - (6 pages) the 2-page application plus covered repairs and bid requirements, contractor selection tips, and Frequently Asked Questions (FAQs)
Application checklist - a checklist of possible documents and forms you may need to complete your REVIVE application packet.
Contractor Selection Confirmation - after you have obtained your contractor estimates, the homeowner chooses which contractor(s) to do each type of home repair work selected on your application. This must be signed and dated in order to be accepted.
Verification of Employment - verifies hire-date, last raise-date, etc. for the job you are currently or were previously working. Contains attached instructions on how to fill it out and where to send it. Instructions are different depending on if you are sending it to a current employer versus a previous employer.
Zero Income Affidavit - for household members 18 or older who are not employed or receiving income of any kind. If you are receiving Public Assistance in the form of food stamps or Medicaid (no checks), you will need to fill out this form. You do not have to claim your food stamps amount as income.

Contractor List - A compiled listing of local contractors. These contractors are not affiliated with Compassionate Care Network, Inc (CCN Inc). We do not recommend nor refer contractors. As the applicant for a Revive grant, you assume all risk and liability. All applicants are responsible for making sure the contractor is licensed and insured, for obtaining and reviewing the estimate, and for selecting the contractor.

Contractor Guidelines for Estimates - give this to each contractor for help in itemizing the estimate. If the estimate is formatted incorrectly, it will be sent back to be re-written, thereby causing a delay in your application submission. THIS IS ESPECIALLY HELPFUL FOR ROOFING CONTRACTORS.
